That would be Ketcham & McDougall, New York City. They made a lot of thimbles.

Sample of Ketcham & McDougall number eight (#8) showing extreme wear of the "K" and Sterling word-mark, but still identifiable:
Search refs.: ornate "MD", "M?D", "MKD", "KMD". Could be mis-read as "WD".
